Hunter Recruitment Group is partnering with KKCS to recruit a Non-Clinician Care Partner to join their Support at Home team.

This is a outstanding opportunity for someone with experience across aged care, community services, care coordination or Home Care Packages/Support at Home who enjoys the organisation, relationships and problem-solving behind coordinating quality care.

About the Role

KKCS has recently introduced a collaborative pod model within their Support at Home team.

As a Non-Clinician Care Partner, you'll work side-by-side with an Enrolled Nurse Care Partner, supported by Registered Nurse clinical oversight, to coordinate services for a shared group of older people living within the community.

Your focus will sit predominantly across the coordination and administration of care, keeping services organised, monitoring budgets, maintaining documentation, communicating with consumers and families and ensuring each person's supports continue to meet their individual needs and goals.

While predominantly office based, you'll also have opportunities to meet consumers in their homes, assist with onboarding and complete reviews.

What You'll Be Doing

- Coordinate services and supports under the Support at Home program
- Support the onboarding of new consumers and ongoing service planning
- Monitor consumer budgets and funding
- Maintain accurate consumer records, documentation and support plans
- Coordinate services in line with individual needs, goals and funding




- Build strong relationships with consumers, families, carers and service providers
- Complete reassessments and appropriate non-clinical reviews
- Support compliance with aged care legislation, quality standards and KKCS processes
- Contribute to continuous improvement across the Support at Home service

About You

You'll bring

- Certificate III in Individual Support (Ageing) or equivalent
- Experience within aged care, community care, care coordination or a similar environment
- Strong organisation and administrative skills
- Confidence managing documentation, budgets and competing priorities
- Strong communication and relationship-building skills
- Confidence using client management systems and general computer applications
- Current Driver's Licence and access to a reliable, registered and comprehensively insured vehicle
- Current First Aid and CPR
- Ability to meet KKCS pre-employment screening requirements

Highly regarded:

- Previous Home Care Package or Support at Home experience
- Previous Care Partner/Care Coordinator experience
- Knowledge of aged care funding, budgets and quality requirements
