Across the state, we are dedicated to offering exceptional care, accommodation, and support services for older Western Australians.About the Role: Juniper is seeking experienced Registered Nurses to join our Home and Community Care team supporting clients across the Perth metro area.In this role, you will provide clinical care and nursing support to clients living independently in the community.
You will conduct assessments, develop and review care plans, and work collaboratively with clients, families and healthcare professionals to ensure safe, high-quality care at home.Juniper is currently seeking 2x Community Registered Nurses to join the team.
Please see position details:1x casual contract; the successful candidate must be available Wednesdays and 1 weekend day a week.1x permanent part-time contract; the successful candidate must be available Monday-Wednesday 8am-4.30pm.Key Responsibilities:Conduct clinical assessments and respond to changes in client health and wellbeingDevelop, implement and review person-centred care plans with clients and their familiesProvide clinical care including medication management,
