About the Role
We are seeking an experienced, reliable and compassionateDisability Support Workerto support a participant living inCheltenham, Victoria, at an SDA property.
This is a high-responsibility support role working with a participant living withMultiple Sclerosis (MS)who is wheelchair and bed bound and requires anindwelling catheter and two staff for transfers.
The participant requires consistent and attentive support with personal care, continence and catheter care, mobility, positioning, pressure care, medication assistance, mealtime management and daily living activities.
The successful candidate must be confident supporting complex physical needs and be able to consistently follow participant-specific support plans, manual-handling procedures, medication requirements, mealtime-management guidelines and escalation protocols.
This position is suited to a calm, observant and safety-focused support worker who understands the importance of safe manual handling, structured routines, accurate documentation and continuity of care.
Key Responsibilities
Provide respectful personal care, including toileting, continence support, dressing and personal hygiene.
Provide catheter care in accordance with the participant's support plan and relevant procedures.
Support a participant who is wheelchair and bed bound with daily living activities and personal care.
Performtwo-person manual handling and transferssafely and in accordance with the participant's manual-handling plan.
Safely use hoists,



slings and other transfer equipment where applicable and in accordance with training and participant requirements.
Support safe positioning, repositioning and pressure-care requirements.
Assist with wheelchair and bed-bound care, ensuring the participant's comfort, dignity and safety.
Provide medication assistance in accordance with the participant's support plan and ACCORD CARE procedures.
Provide mealtime assistance in accordance with the participant's mealtime-management requirements.
Observe changes in the participant's physical condition and promptly elevate concerns in accordance with established procedures.
Follow participant-specific support plans and clinical guidance consistently.
Complete clear and accurate progress notes, incident reports and handover documentation.
Maintain a safe, respectful and well-organised support environment.
Work collaboratively with the participant, family, support team, clinical staff and allied health professionals.
Follow ACCORD CARE policies, NDIS Practice Standards and all participant-specific support plans.
Skills and Experience
Minimum2 years' experience in disability and/or aged careis preferred.
Previous experience supporting people withMultiple Sclerosis (MS)is highly regarded.
Demonstrated experience supporting people with complex physical support needs.
Experience providing catheter care.
Experience with two-person manual handling and transfers.
Confidence using hoists, slings and other transfer equipment where applicable.
Experience supporting wheelchair and bed-bound participants.
Strong understanding of positioning and pressure-care requirements.
Experience providing personal care,



toileting and continence support.
Experience providing medication assistance.
Experience with mealtime management.
Strong observation, documentation and escalation skills.
A respectful, patient and person-centred approach.
Mandatory Requirements
Certificate III or IV in Individual Support, Disability or a related qualification, or equivalent relevant experience.
Current NDIS Worker Screening Check.
Current Working with Children Check, where required.
Current First Aid and CPR certificates.
Current medication-assistance training.Current manual-handling and hoist-transfer training or demonstrated relevant experience.
Valid Australian driver licence.
Evidence of current Australian work rights.
Willingness to complete participant-specific training and competency assessments before commencing independent shifts.
Ability to safely undertaketwo-person transfers and manual handlingas required.
Desirable Experience
Experience supporting participants withMultiple Sclerosis (MS).
Previous catheter-care experience.
Experience supporting wheelchair and bed-bound participants.
Experience with pressure care and positioning.
Experience performing two-person manual handling and transfers.
Experience using hoists, slings and other transfer equipment.
Experience working in Supported Independent Living or SDA environments.
Experience supporting people with complex physical support needs.
Previous experience working for an NDIS provider.
