is seeking compassionate, reliable and experienced
Registered Nurses
to join our growing community nursing team across
Perth Metro and the Peel region
.

The successful candidates will provide high-quality, person-centred nursing care to clients in their homes, supporting their health, independence and wellbeing.
Key Responsibilities n
n
Conduct nursing assessments and monitor clients' health and wellbeing.
n
Medication administration and management.
n
Wound care and dressing changes.
n
Catheter, continence, diabetes and chronic disease support.
n
Monitor vital signs and recognise changes in clients' conditions.
n
Escalate clinical concerns appropriately.
n
Provide health education and support to clients and families.
n
Maintain accurate and timely clinical documentation.
n
Liaise with GPs, allied health professionals and other healthcare providers.
n




Follow infection control, clinical governance and workplace safety requirements.
n
Essential Requirements n
n
Current
AHPRA registration as a Registered Nurse
.
n
Current National Police Clearance.
n
Current First Aid and CPR.
n
Current Australian Driver's Licence and reliable vehicle.
n
Strong clinical assessment, communication and documentation skills.
n
Ability and willingness to provide community-based services across
Perth Metro and Peel areas
.
n
Community nursing, home care, aged care or hospital experience is highly regarded.
n
