ACH Group in Adelaide is seeking a passionate Community Connector to lead the Social Experiences team and support older people to build meaningful social connections. This full time role is based at Elizabeth on Mondays and across ACH Group locations for the rest of the week.
You will develop individual social plans, facilitate engaging group activities, and help create inclusive social prospects that enhance wellbeing and reduce social isolation.
