What you will do
n
n
Deliver high-quality clinical pharmacy services across hospital wards.
n
Conduct comprehensive medicines reconciliation and medication history assessments.
n
Perform clinical screening of prescriptions and drug charts to ensure patient safety.
n
Complete TTA screening and detailed discharge medication reviews.
n
Provide specialist pharmaceutical advice to multidisciplinary healthcare teams.
n
Support safe and effective medicines optimisation across the service.
n
Review and develop medication therapy plans to improve patient outcomes.
n
Participate in ward rounds and contribute to clinical decision‐making processes.
n
Ensure adherence to medicines management policies, procedures, and best practice guidance.
n
Maintain accurate clinical documentation and pharmacy records.
n

Requirements
n
n
GPhC Registration: Qualified Pharmacist with current General Pharmaceutical Council registration.
n
NHS Experience: Proven NHS experience, particularly within hospital ward-based pharmacy services.
n
Medicines Reconciliation: Experience completing drug histories and medicines reconciliation.
n
Clinical Screening: Strong experience with clinical screening and TTA reviews.
n
EPMA Knowledge: Familiarity with EPMA systems, preferably Cerner EPMA.
n
Clinical Expertise: Excellent clinical pharmacy knowledge and decision-making skills.
n
Communication Skills: Robust interpersonal, organisational, and multidisciplinary working skills.
n
Band 7 or Band 8a Experience: Previous experience working at Band 7 or Band 8a level.
n
Acute Hospital Experience: Experience within an acute hospital pharmacy environment.
