nOn behalf of our client - a community-focused organization dedicated to empowering individuals with disabilities and mental health challenges - we are seeking a compassionate Youth Worker. The role centers on person-centered support, fostering independence, and connecting young people to community, education, and employment pathways.n

nKey Duties and Responsibilitiesn
nAssess and Plan Supportn
n• Evaluate clients' social, emotional, and physical needs to co-design individualized support and goal plans.n
n• Develop and implement educational, training, and life-skills programs that promote self-advocacy and community inclusion.n
n
nEngage and Monitor Progressn
n• Conduct strengths-based interviews to understand each young person's aspirations and barriers.n
n• Regularly review and report on client progress, adjusting strategies to ensure meaningful outcomes.n
n
nRefer and Liaisen
n• Connect clients with specialist services – including housing, health, and employment agencies – to address complex needs.n
n• Build partnerships with community groups, government bodies,



and local businesses to enhance resource access and awareness.n
n
nSupport Families and Vulnerable Groupsn
n• Provide family education and crisis support, particularly for young people with disabilities or those in out-of-home care.n
n• Assist in adult service units and group housing, focusing on skill development and independent living.n
n
nCommunity Advocacy and Fundingn
n• Identify community gaps in welfare, housing, and recreation services, and advocate for inclusive resources.n
n• Prepare funding submissions and reports for government and philanthropic bodies to sustain and expand programs.n
n
nRequirementsn
n• Relevant qualification in youth work, community services, or social work.n
n• Minimum 1-year full time experience in a related position.n
n• Experience in disability or mental health settings, with strong case management and communication skills.n
n
