As a Care Partner, you'll build meaningful relationships, coordinate personalised services, and support older people to thrive in their own homes across Alstonville and Tweed Heads.

How You'll Make an Impact
n
n
Partner with clients, families, and key stakeholders to deliver personalised care that supports independence, wellbeing, and choice.
n
Manage a portfolio of Home Care clients, ensuring care plans remain current, outcomes-focused, and aligned with individual needs, goals, and funding.
n
Conduct regular client assessments and reviews to identify changing needs and coordinate appropriate supports.
n
Build strong relationships with clients and care teams to ensure high-quality, person-centred services are delivered with compassion and excellence.
n

Our Ideal Candidate
n
n
Qualified in Individual Support, Ageing Support, Case Management, or a related field.
n
Experienced in care coordination, client assessments, and leading care teams.
n
Knowledgeable in CHSP and Support at Home programs.
n
Confident using Microsoft Office and digital care management systems.
n
An excellent communicator with solid organisational and problem-solving skills.
n
Holds a current Driver Licence, NDIS Worker Screening Check, and First Aid Certificate.
n
Passionate about delivering exceptional care and supporting older people to live independently.
n

We are looking for someone on a permanent part time basis available Monday to Wednesday in Alstonville.
