Job Description
ATSICHS Brisbane is seeking a dedicated Care Coordinator Nurse to support patients with chronic illness.
You will provide care coordination and work closely with clients to improve health outcomes in Aboriginal and Torres Strait Islander communities.
The role requires a Bachelor in Nursing and full unconditional AHPRA registration, with solid communication and collaboration skills.
This is a permanent full-time position in Brisbane, offering a starting salary around AUD 78,447 plus 12% super.
