Caring Approach is a dynamic, vibrant, supportive, and caring organization. We provide tailored in-home support and community care for the elderly, people with both physical and intellectual disabilities, and people needing short-term assistance after being discharged from the hospital.n

nWe are seeking caring and qualified individuals to become part of the Caring Approach team to promote independence for people living at home.n
n
nGeneral duties include:n
n
nAssess participant needs, goals, abilities and support requirements in line with their NDIS plans.n
nAssist in developing and implementing individualised support plans and community access programs.n
nSupport participants with social, emotional, intellectual and physical disabilities to build independence and life skills.n
nMonitor participant progress and recommend adjustments to support strategies where required.n
nLiaise with families, guardians, support coordinators, allied health professionals and other service providers.n
nProvide guidance to disability support workers in implementing participant support plans.n
nPromote participant choice, control, dignity, privacy and community inclusion.n
nIdentify and report risks, incidents, changes in participant needs and safeguarding concerns.n
nMaintain accurate case notes, progress reports and service delivery records.n




nEnsure services are delivered in accordance with NDIS Practice Standards, organisational policies and relevant legislation.n
nSupport participants to access education, recreation, social, employment and community activities.n
nParticipate in team meetings, case reviews, training and continuous improvement activities.n
n
nTo be successful in this role you will need:n
n
nRelevant qualification in disability, community services, individual support, health or a related field.n
nPrevious experience working with people with disability, preferably in an NDIS environment.n
nStrong understanding of person-centred practice, participant rights and duty of care.n
nAbility to develop, implement and monitor individualised support strategies.n
nGood written communication skills and ability to maintain accurate records.n
nAbility to work with participants with complex behaviours or high support needs.n
nStrong communication and interpersonal skills.n
nCurrent NDIS Worker Screening Check or willingness to obtain one.n
nCurrent First Aid and CPR certificates or willingness to obtain them.n
nDriver licence and access to a reliable vehicle may be required.n
