Launch Housing is seeking a part-time Case Manager for the Supporting Families Program in Melbourne. You will deliver intensive outreach-based case management to families with children, developing tailored plans and coordinating services to maintain safe housing.

You will engage with clients to assess eligibility, build strengths-based plans, and connect them with education, health and community resources. A driver's licence and child-secure practice are required.
