The Allied Health Chronic Disease Intermediate Care Service (ICS) comprises of Physiotherapy, Dietetics, Exercise Physiology, Occupational Therapy, Podiatry, Social Work and Interdisciplinary Complex Self-Management support services, operating as an intermediate service between hospital admission and community supports.
Our team works collaboratively, and in alignment with other services within NALHN Clinical Divisions, as well as local Aboriginal Health Services, with a focus on optimising functional and health outcomes and reducing hospital admissions for clients with Diabetes and Chronic Cardiac and Respiratory Conditions.

As a Physiotherapist with our team, you'll have the opportunity to make a real difference to people living with chronic lung conditions.
Working predominantly in pulmonary rehabilitation, you'll support people who may be living with significant breathlessness, reduced exercise capacity and the challenges that come with a chronic respiratory condition.
Through education, exercise, self-management support and support with airway clearance techniques, you'll work alongside clients to build their confidence, improve their quality of life and achieve the goals that matter to them.
This part-time contract role is based across Adelaide's northern metropolitan area and is an opportunity to work as part of a supportive, collaborative multidisciplinary team where your skills, ideas and contribution are valued.
What You'll Be Doing You'll work with people living with chronic respiratory conditions to improve their physical capacity, confidence and ability to manage their condition in everyday life.
Working predominantly within our pulmonary rehabilitation program, you'll combine your physiotherapy skills with education and self-management support to help people understand and manage their respiratory condition.
You'll work alongside each person to identify what is key to them and support them to take meaningful steps towards their goals.
You'll work across community-based settings in Adelaide's northern metropolitan area, providing care that is accessible, practical and focused on helping people manage their health closer to home.
This is a part-time, Monday to Friday, fixed-term contract through to 31 July **** .
Day-to-day, your contribution will include: Conducting comprehensive respiratory and functional assessments and developing individualised, person-centred goals and care plans, inclusive or airway clearance and exercise therapies Delivering pulmonary rehabilitation, including exercise assessment, prescription and progression, tailored to each person's abilities, needs and goals.
Providing education and self-management support to build confidence in managing chronic respiratory conditions, breathlessness and other health challenges.
Supporting people to improve exercise capacity, physical function, confidence and quality of life.
Recognising and responding to the impact of co-morbidities, psychosocial factors and other barriers on a person's health and wellbeing.
Working collaboratively within a multidisciplinary allied health team, alongside GPs, Respiratory specialists and nurses, Aboriginal Health Services and other NALHN services.
Connecting people with other services and supports when additional needs are identified.
Maintaining accurate clinical documentation and contributing to quality improvement in accordance with SA Health standards.
What You'll Bring Tertiary qualification in Physiotherapy and current AHPRA registration.
Experience in community or primary health care settings.




Sound understanding of chronic disease management and functional rehabilitation.
Ability to work autonomously while contributing to a collaborative team workplace.
Familiarity with SA Health clinical systems and documentation requirements.
To be eligible for this position, you must have an appropriate degree or equivalent qualification which entitles registration with the Physiotherapy Board of Australia.
