Southern Adelaide Local Health Network – Daw Park - Geriatric Outreach Service Salary $112,****** - $120,****** p.a. (Pro Rata) AHP3 | Part Time Contract until August ****, 30 hours per week We are seeking an experienced and motivated Occupational Therapist to join the Division of Rehabilitation, Aged Palliative Care who is passionate about working with older people and managing complex clinical presentations About the role: The Senior OT role works as part of the Geriatric Outreach Service, working closely with the Geriatricians and Nursing team to deliver care to patients within residential aged care facilities and prevent unnecessary hospital transfers.
The role supports patients with dementia, memory loss and associated Behavioural and Psychological Symptoms of Dementia (BPSD) to develop and implement patient centred management plans that support wellbeing and optimise quality of life.
Duties for the role will include: Assessing patient's occupational performance, cognitive function, behavioural presentation, strengths, needs and environmental factors using a range of formal and informal assessments Participating in interdisciplinary clinical and planning activities to enhance the overall service provided to patients Providing Occupational Therapy services to RACF In-Reach patients including use of a broad range of technologies as required About you: A degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia Demonstrated high level interpersonal skills and ability to establish and maintain effective networks with key stakeholders Experience as a clinician in the assessment and provision of high-quality Occupational Therapy services to clients with a variety of conditions requiring management of issues associated with cognitive impairment and/or changed behaviour Experience in providing an approach to care that focuses on behaviour support initiatives and non-pharmalogical approaches Why SALHN?
