About the Role n
We are seeking an experienced, reliable and compassionate Disability Support Worker to support a participant living in Cheltenham, Victoria , at an SDA property.
n
This is a high-responsibility support role working with a participant living with Multiple Sclerosis (MS) who is wheelchair and bed bound and requires an indwelling catheter and two staff for transfers . The participant requires consistent and attentive support with personal care, continence and catheter care, mobility, positioning, pressure care, medication assistance, mealtime management and daily living activities.
n
The successful candidate must be confident supporting complex physical needs and be able to consistently follow participant-specific support plans, manual-handling procedures, medication requirements, mealtime-management guidelines and escalation protocols.
n
This position is suited to a calm, observant and safety-focused support worker who understands the importance of safe manual handling, structured routines, accurate documentation and continuity of care.
Key Responsibilities n
n
- Provide respectful personal care, including toileting, continence support, dressing and personal hygiene.
n
- Provide catheter care in accordance with the participant's support plan and relevant procedures.
n
- Support a participant who is wheelchair and bed bound with daily living activities and personal care.
n
- Perform two-person manual handling and transfers safely and in accordance with the participant's manual-handling plan.
n
- Safely use hoists,
slings and other transfer equipment where applicable and in accordance with training and participant requirements.
n
- Support safe positioning, repositioning and pressure-care requirements.
n
- Assist with wheelchair and bed-bound care, ensuring the participant's comfort, dignity and safety.
n
- Provide medication assistance in accordance with the participant's support plan and ACCORD CARE procedures.
n
- Provide mealtime assistance in accordance with the participant's mealtime-management requirements.
n
- Observe changes in the participant's physical condition and promptly elevate concerns in accordance with established procedures.
n
- Follow participant-specific support plans and clinical guidance consistently.
n
- Complete clear and accurate progress notes, incident reports and handover documentation.
n
- Maintain a safe, respectful and well-organised support environment.
n
- Work collaboratively with the participant, family, support team, clinical staff and allied health professionals.
n
- Follow ACCORD CARE policies, NDIS Practice Standards and all participant-specific support plans.
n
Skills and Experience n
n
- Minimum 2 years' experience in disability and/or aged care is preferred.
n
- Previous experience supporting people with Multiple Sclerosis (MS) is highly regarded.
n
- Demonstrated experience supporting people with complex physical support needs.
n
- Experience providing catheter care.
n
- Experience with two-person manual handling and transfers.
n
- Confidence using hoists, slings and other transfer equipment where applicable.
n
- Experience supporting wheelchair and bed-bound participants.
n
- Strong understanding of positioning and pressure-care requirements.
n
- Experience providing personal care, toileting and continence support.
n
- Experience providing medication assistance.
n
- Experience with mealtime management.
n
- Strong observation, documentation and escalation skills.
n
- A respectful, patient and person-centred approach.
n
Mandatory Requirements n
n
- Certificate III or IV in Individual Support, Disability or a related qualification, or equivalent relevant experience.
n
- Current NDIS Worker Screening Check.
n
- Current Working with Children Check, where required.
n
- Current First Aid and CPR certificates.
n
- Current medication-assistance training.Current manual-handling and hoist-transfer training or demonstrated relevant experience.
n
- Valid Australian driver licence.
n
- Evidence of current Australian work rights.
n
- Willingness to complete participant-specific training and competency assessments before commencing independent shifts.
n
- Ability to safely undertake two-person transfers and manual handling as required.
n
Desirable Experience n
n
- Experience supporting participants with Multiple Sclerosis (MS) .
n
- Previous catheter-care experience.
n
- Experience supporting wheelchair and bed-bound participants.
n
- Experience with pressure care and positioning.
n
- Experience performing two-person manual handling and transfers.
n
- Experience using hoists, slings and other transfer equipment.
n
- Experience working in Supported Independent Living or SDA environments.
n
- Experience supporting people with complex physical support needs.
n
- Previous experience working for an NDIS provider.
