Right at Home Newcastle Toronto provides in-home care to older people across Western Lake Macquarie, and we are looking for a Care Partner to join us full time.

The Care Partner, also called a Care Coordinator, is the person a client deals with most. You take new clients through onboarding, work out what they need, put the right services in place, and then stay with them as their situation changes. Your week is a mix of time in the office and time out on the road visiting people at home.

Support at Home is still new, and for a lot of older people and their families it is confusing. A positive part of this job is making sense of it for them, so they understand what they are entitled to and feel confident about the decisions they are making. If you like that side of the work, you will enjoy this role.

What you would be doing:

Onboarding new clients and carrying out non-clinical assessments using our standard tools

Building care plans with clients and their families, and reviewing them as things change

Putting services in place and making sure the roster works for your clients

Keeping an eye on how your clients are going, and raising anything clinical with our nursing team

Working alongside GPs, allied health and other providers

Being the point of contact for families, referrers and our care workers

Handling service enquiries and taking prospective clients through what we offer

Keeping records up to date and making sure billing lines up with the care plan

Supporting and mentoring our care workers

Occasionally stepping in to cover a care shift when the roster needs it

This is a full time role, 38 hours a week. Pay is under the SCHADS Award (MA000100) and set to your level of experience.

Knowledge of the aged care and community care sector, including the Aged Care Quality Standards, the Code of Conduct for Aged Care, and the rights of the people in our care The ability to assess, plan, arrange and review care and services for clients at home

Strong written and verbal communication

Confidence talking with clients and their families, and with GPs, allied health and our own care team A clear sense of how the Registered Nurse, Enrolled Nurse, personal care worker and companion care worker roles fit together A solid grasp of keeping clients, yourself and others safe, including emergency procedures and work health and safety

Strong time management, and the ability to run your own day across visits, calls and paperwork

Confidence using Microsoft Office and CRM or rostering systems

Good documentation habits

Willingness to lead by example with our care workers and pitch in across the team

Certificate III in Individual Support (Ageing or Disability), or an equivalent care qualification

Experience in aged care or home care

Familiarity with the Support at Home program, including budgets, quarterly funding and care management

Experience carrying out non-clinical assessments and writing care plans A higher qualification in aged care, community services or management

Additional training in dementia care

Experience supporting care transitions, such as hospital discharge planning

Knowledge of funding streams, quality standards and audits

Experience supervising or mentoring care workers

Experience handling service enquiries and talking prospective clients through their options

- Patience and warmth with older people and their families, including in the harder conversations
