Job Description

Sunrise Health Service seeks a passionate Diabetes Educator to join the Chronic Care team. You will deliver culturally secure education and care across Sunrise communities through regular outreach visits and collaborate with multidisciplinary teams.

You will support diabetes self-management, early intervention, and improved long-term outcomes while contributing to coordinated care planning and building local capacity in remote settings.
