Support children and young people in Out-of-Home Care through case management, therapeutic planning, and frontline coaching. Drive positive outcomes by embedding trauma-informed, culturally safe practices and ensuring compliance with standards and legislation.

The role operates across two primary functions: n

- 18 hours per week supporting frontline staff in the homes to implement case and care strategies.
n
- 20 hours per week completing case work duties, including planning, documentation, and stakeholder engagement.

What You'll Be Doing: nn
- Lead and review case plans to meet therapeutic and cultural goals
n
- Coach and support staff to embed strategies in daily care
n
- Engage with families, carers, and agencies to align goals
n
- Maintain documentation and ensure compliance with standards
n
- Promote safe, nurturing,



and culturally respectful environments

What You'll Bring: nn
- Diploma (or working towards) in community, youth, or social work
n
- Proven case management and stakeholder engagement skills
n
- Knowledge of trauma-informed and therapeutic care practices
n
- Solid documentation and compliance capability
n
- Current full Australian driver's licence and probity clearances
n
