The LPN works under the direction of a Registered Nurse to deliver patient-centered care, support treatment plans, monitor patient conditions, and assist with admissions, discharges, and patient education.Position Details: n

Key Responsibilities

Provide direct patient care within the LPN scope of practice.

n
- n

Assist with patient admissions, discharges, assessments, and care planning.

n
- n

Monitor patient conditions and promptly communicate changes to the RN.

n
- n

Participate in bedside shift reports, patient rounding, and interdisciplinary collaboration.

n
- n

Reinforce patient and family education.

n
- n

Delegate and support patient care activities for unlicensed assistive personnel as appropriate.

n
- n

Maintain accurate documentation and follow hospital policies and procedures.

n
- n

Complete required competencies, certifications, and ongoing education.

n
- n

Contribute to quality improvement initiatives and department goals.

n

Qualifications

n

n
- n





Graduate of an accredited Practical Nursing program.

n
- n

Current LPN license in the state of practice.

n
- n

Recent graduates and experienced LPNs encouraged to apply.

n
- n

Strong communication, teamwork, and patient care skills.

n

n

Certifications

n

n
- n

BLS (American Heart Association) required within 30 days of hire.

n
- n

IV Certification required upon hire or within 3 months of hire, based on department requirements.

n
- n

Additional certifications (ACLS, PALS, PEARS, S.T.A.B.L.E., or other unit-specific certifications) may be required based on assignment.
