Make a meaningful difference for people living with complex mental health needs through the Pathways to Community Living Initiative. Join our team and use your clinical expertise and leadership skills to support people transitioning from long-term inpatient care back into the community.

About The Opportunity

Join the Central Coast Local Health District’s Pathways to Community Living Initiative (PCLI) and play a key role in supporting people with enduring and serious mental illness to transition from long-term inpatient care back into the community. Working across inpatient and community settings, you will provide specialist clinical leadership while contributing to service development, care planning and the implementation of the PCLI framework. In this role you will:

- Work collaboratively with Mental Health and AOD teams to identify consumers requiring PCLI support and develop individualised assessment, care and transition plans.
- Provide specialist mental health intervention, clinical leadership and expert consultancy across inpatient and community settings.
- Support consumers, families and carers through a recovery-oriented approach, working closely with internal and external stakeholders.
- Contribute to the development, review and implementation of clinical processes, policies, procedures and new models of care.
- Provide leadership, supervision and mentoring to PCLI team members while contributing to quality improvement, service planning and the ongoing evaluation of the PCLI program.

About You

You will be an experienced mental health professional with strong clinical expertise, leadership capability and a passion for supporting people with complex mental health needs to achieve positive outcomes in the community. You will bring a collaborative, recovery-oriented approach and be confident working across multidisciplinary teams and with consumers, families,



carers and external stakeholders. We are looking for someone who has:

- Current AHPRA registration as a Registered Nurse, Senior Psychologist or Occupational Therapist, or eligibility for AASW membership as a Social Worker.
- Demonstrated clinical expertise in mental health, including experience providing complex care and working within a recovery-oriented framework.
- Strong leadership, communication and interpersonal skills, with the ability to mentor and support multidisciplinary teams.
- Experience developing and implementing innovative models of care, clinical processes and service improvements for people with complex needs.
- A strong understanding of contemporary mental health practice, including relevant NSW Health policies, standards and the Pathways to Community Living Initiative (PCLI).
