About the role

Aequra Residences is seeking an experienced Clinical Care Coordinator to support clinical quality, staff capability and regulatory compliance at our Punchbowl residence.

Reporting to and working closely with the Care Manager, the Clinical Care Coordinator will primarily coordinate staff education, competency assessment, clinical auditing and continuous improvement activities.

KEY RESPONSIBILITIES

Education and staff capability

- Assess the education needs of clinical and care employees.
- Develop, coordinate and deliver education programs
- Coordinate clinical orientation and induction for new clinical and care employees.
- Conduct and document competency assessments within the employee’s role and scope of practice.
- Maintain accurate education records.
- Monitor mandatory training compliance and follow up outstanding requirements.
- Provide coaching and practical support to employees.

Auditing and compliance

- Conduct audits of clinical documentation, care plans, assessments, medication management, infection prevention and control, incident management, restrictive practices and other relevant care processes.

- Analyse audit findings
- Support the development,



implementation and monitoring of corrective-action and continuous-improvement plans.
- Assist with preparations for Aged Care Quality and Safety Commission audits, assessment contacts and other regulatory activities.
- Support compliance with the Aged Care Act 2024, the strengthened Aged Care Quality Standards, the Aged Care Rules 2025 and organisational policies and procedures.
- Assist with maintaining evidence demonstrating the residence’s compliance with applicable regulatory requirements.

Quality improvement and clinical data

- Review incidents, complaints, feedback, clinical indicators and audit results
- Support the investigation and review of clinical incidents
- Prepare clinical quality reports and present findings to the Care Manager and management team.
- Support the implementation and evaluation of new clinical procedures, systems and improvement initiatives.

Support to the Care Manager

- Assist the Care Manager with clinical oversight and resident-care matters as required.
- Support reviews of care plans, assessments and clinical documentation
- Provide guidance to clinical and care employees in consultation with the Care Manager.
- Assist with the monitoring and follow-up of identified clinical risks.




- Support communication with residents, families, representatives and health professionals when delegated by the Care Manager.

QUALIFICATIONS AND EXPERIENCE

Essential requirements:

- Current unrestricted registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency.
- A minimum of five years’ experience as a Registered Nurse in residential aged care.
- At least three years’ experience in a Clinical Care Coordinator role or a comparable position with substantial responsibility for clinical education, auditing and continuous improvement.
- Robust knowledge and demonstrated practical application of the Aged Care Act 2024, the strengthened Aged Care Quality Standards and Aged Care Quality and Safety Commission requirements.
- Demonstrated experience developing and delivering education and competency-assessment programs
- Demonstrated experience conducting clinical audits, analysing findings and monitoring corrective actions.
- Ability to work collaboratively with and provide effective support to the Care Manager and broader management team.
- Strong digital literacy and experience using electronic care-management and learning-management systems.

Desirable requirements:

- Qualifications in workplace training and assessment, clinical education, aged care, infection prevention and control or quality management.
- Experience providing clinical support in a complex residential aged care environment.
