Shape Better Outcomes Beyond the Hospital Join the CHIP team and use your clinical expertise to support hospital avoidance, care coordination and better outcomes for patients in the community. About This Role

Your new role: As a Clinical Nurse with the Community Hospital Interface Program (CHIP), you will provide advanced clinical care, assessment, planning and coordination for patients with complex needs. Working across Princess Alexandra and QEII Hospitals, you'll play a key role in supporting hospital avoidance, reducing length of stay and connecting patients with appropriate community services.

You'll work autonomously while providing clinical leadership, education and support to the wider healthcare team.

- Provide advanced clinical assessment, care planning and coordination for complex patients
- Lead the delivery of holistic, person-centred care within a multidisciplinary team
- Coordinate referrals and connect patients with appropriate community and aged care services
- Provide clinical leadership, mentoring, education and support to nursing and healthcare staff

About You We are looking for an experienced Registered Nurse who brings advanced community nursing knowledge, strong clinical judgement and a passion for supporting patients to receive care in the most appropriate setting. You will be a confident communicator and collaborative leader who can build effective relationships with patients, families, carers and healthcare professionals.

- Demonstrate advanced clinical assessment, critical thinking and problem-solving skills




- Demonstrate strong knowledge of discharge planning and hospital avoidance strategies
- Build effective relationships across multidisciplinary teams and community services
- Provide clinical leadership through delegation, education, mentoring and role modelling
- Communicate sensitively and effectively with patients, families, carers and stakeholders
