About Uprety Home Care

Uprety Home Care provides nursing services to people living in their own homes, including NDIS participants, older people and DVA clients. Our work includes complex wound care, continence assessments and catheter care, alongside support for a range of ongoing clinical needs.

We are seeking experienced Registered Nurses for permanent part-time positions that will work across Lake Macquarie and Newcastle

About the role

You will visit clients at home, assess their nursing needs, develop and review individualised care plans, and deliver care within your scope of practice.

This role combines direct client care with clinical documentation, assessment reports and follow-up. You will work with our administration and operations teams to coordinate your workload and communicate clearly with clients, families and other healthcare professionals.

Your responsibilities

- Complete nursing assessments and develop, implement and review person-centred care plans.
- Provide wound assessments, wound management and catheter care, including SPC and IDC changes where clinically appropriate and within your competence.
- Complete continence assessments and prepare reports with clear findings, recommendations and consumables.
- Provide nursing care to NDIS, aged-care and DVA clients.
- Maintain accurate, timely progress notes, care plans and clinical records.
- Monitor changes in clients’ needs and seek clinical advice or make referrals when required.
- Educate clients and their carers about relevant aspects of their care.
- Plan your workload,



meet agreed deadlines and communicate any barriers to timely completion.
- Travel between clients within your allocated service area.

Experience with PEG Tube changes, diabetes management, stoma care and pressure injury prevention and management will also be valued.

About you

You are comfortable working independently in clients’ homes, while recognising when to seek advice or involve other healthcare professionals. You combine sound clinical judgement with clear communication and reliable organisation.

You will bring

- Current AHPRA registration as a Registered Nurse.
- Relevant community nursing experience of at least 5 years, or experience that demonstrates readiness to work on your own.
- Substantial Experience in complex wound care, SPC and IDC catheter management and changes.
- Robust assessment, care-planning and clinical documentation skills.
- The ability to manage an assigned caseload and complete reports and follow-up within agreed timeframes.
- A current driver licence and access to a reliable vehicle.
- Australian working rights and willingness to complete relevant pre-employment screening.

Experience preparing continence assessment reports for NDIS and aged-care clients is required. Relevant postgraduate qualifications are welcomed.

Working arrangements

- Permanent part-time employment.
- 22.8 hours per week.
- A combination of client visits, travel, care planning, report writing and follow-up.
- Potential for additional full working days, subject to service needs and agreement.
- Reporting and Desk work is completed remotely, as Work from Home.
