Extended Post-Care Support Worker
This role is full time 38 hrs per week.
Salary is in line with the Social, Community, Home Care & Disability Services Industry Award 2010- Social and Community Services Employee.

Beenleigh Housing and Development Company Ltd (BHDC) is the oldest Aboriginal and Torres Strait Islander Community Controlled Organisation of its type in Eagleby, Beenleigh Logan QLD. BHDC strives to support, enhance, and transform the lives of its members in culturally supportive, community connected and regenerative ways. BHDC was formally established in 1994 as a not-for-profit, registered charity holding Public Benevolent Institution (PBI) registration with the Australian Taxation Office.
Jinndi Mibunn BHDC is dedicated to improving the lives of young people who have recently turned 18 by providing culturally appropriate, proactive, and practical support through our Extended Support Initiative. We are committed to helping these young adults transition into independent living while maintaining strong connections with their culture, family, community, and health and wellbeing.
We are seeking a highly motivated Support Worker for our Extended Support Post Care Initiative. As an Extended Support Worker, you will play a pivotal role in this new initiative focused on providing culturally appropriate and proactive support to young people who have turned 18. This initiative aims to empower young people to transition successfully into independent adulthood by addressing their unique needs, including housing, employment, education, cultural connections, health and wellbeing, and financial stability.
The ideal candidate should have a strong understanding of the needs and challenges faced by young adults in transition and be capable of providing culturally sensitive support.
Key Responsibilities:
Client Support:
- Build strong and trusting relationships with young people aged 18 and above.
- Assess individual needs and develop personalized support plans in collaboration with clients.
- Provide practical assistance in securing protected and affordable housing options.
- Assist in job searches, resume building, and enrolment in training courses.
- Support clients in maintaining connections with their family, culture, country, and community.
- Accompany clients to appointments related to health, wellbeing, and other essential services.
- Assist in accessing financial assistance programs for independent living.
Cultural Competency:
- Demonstrate cultural sensitivity and understanding when working with clients from diverse backgrounds.
- Incorporate cultural considerations into support plans and service delivery.
- Foster an environment of respect for clients' cultural heritage and identities.
Team Collaboration:
- Work closely with the Extended Support Team Leader to ensure coordinated and effective support.
- Collaborate with other support workers to share insights, best practices, and resources.
- Participate in team meetings and contribute to the development of program strategies.
- Documentation and Reporting:
- Maintain accurate and up-to-date client records, progress notes, and case files.
- Generate reports on client outcomes and program impact as required.
- Ensure compliance with all relevant policies, procedures, and regulations.
Continuous Learning:
- Stay informed about community resources, services, and opportunities available to clients.
- Engage in ongoing professional development to enhance skills and knowledge.

Essential Criteria
- Minimum of a high school diploma; bachelor's degree in social work or a related field is preferred.
- Previous experience working with young people, preferably in a support or mentoring capacity.
- Understanding of the challenges faced by young adults transitioning to independence.
- Cultural competence and the ability to work with diverse communities.
- Strong communication, active listening, and problem-solving skills.
- Empathetic, patient, and compassionate approach to client support.
- Ability to work independently and as part of a team.
- Proficiency in record keeping and documentation.
- Valid driver's license and access to reliable transportation.
- Background check will be necessary.
Selection Criteria
- Experience and/or the ability to demonstrate community development skills in Aboriginal and Torres Strait Islander communities, particularly a demonstrated ability to communicate effectively and sensitively with Aboriginal and Torres Strait Islander people.
- High level of communications skills, including the ability to hold difficult and challenging conversations with clients, when required.
- Demonstrated writing skills and the ability to produce comprehensive, detailed, and accurate case notes, reports and assessments using a computer including spreadsheets, email, internet, digital media etc.
- Demonstrated ability to work independently and as part of a team.
