We're seeking passionate and skilled Brief Intervention Workers to join our Housing and Homelessness Team, delivering the Homeless Outreach Access Program (HOAP) and Immediate Housing Response (IHR). This is an opportunity to work alongside vulnerable individuals and families at critical moments in their lives and help them navigate a pathway towards safe and sustainable housing outcomes.
About the Role
As a HOAP Brief Intervention Worker, you will provide outreach, assessment, advocacy, brief intervention, service navigation, and coordinated support to individuals and families experiencing homelessness or housing instability across the Moreton Bay region.
Based across our Lawnton, Redcliffe and Kallangur sites, you will work a rotating roster of:
- Morning Shift: 8:00am to 4:00pm
- Afternoon Shift: 12:00pm to 8:00pm
Working within a strengths-based, trauma-informed, culturally safe and person-centred framework, you will:
- Conduct intake, assessments and brief interventions.
- Develop practical support plans that address immediate needs and support long-term housing outcomes.
- Provide advocacy, referrals, information and practical assistance to access housing, health, legal, financial and community services.
- Undertake assertive outreach with people who are rough sleeping or at risk of homelessness.
- Support clients to navigate housing systems and community support networks.
- Coordinate warm referrals and collaborate with government agencies, community organisations and specialist services.
- Maintain accurate case records, reporting and administrative requirements.
What You'll Bring You're someone who believes everyone deserves a safe place to call home and understands the importance of dignity, respect and inclusion when supporting vulnerable people.
Essential Requirements
- Qualification in Social Work, Human Services, Community Services or a related discipline, or equivalent experience.
- Experience supporting people experiencing homelessness, housing instability or complex social needs.
- Experience in outreach, crisis intervention, case management or brief intervention.
- Strong assessment, engagement, advocacy and service navigation skills.
- Knowledge of homelessness services, housing pathways and community support systems.
- Strong communication and stakeholder engagement skills.
- Ability to work independently and collaboratively.
- Competence using client management systems and Microsoft Office.
- Commitment to strengths-based, trauma-informed and culturally safe practice.
Mandatory Requirements
- Current Queensland Driver Licence.
- Current Working with Children Check (Blue Card), or ability to obtain.
- National Criminal History Check.
- Full Australian working rights.
