We also support people, including older people and people with disability, to stay living at home longer and to live well in their community.

Our broad range of primary health services, care management and in home and community support services can be tailored to an individual’s needs and with major hubs in Box Hill and Eltham, these services can be delivered locally, in the Eastern and North Eastern suburbs of Melbourne.

The Opportunity

We are looking for a Service Coordinator to join our supportive and skilled multidisciplinary teams in a full-time capacity.

Responsibilities include but are not limited to;

- Provide high-quality customer support to Healthy Ageing at Home clients, their families, and the team.
- Coordinate and roster newly onboarded support staff, including arranging buddy shifts to support their transition.
- Manage and schedule client services within appropriate IT systems, ensuring accurate documentation of all actions and communications.
- Follow up promptly on referrals and service requests to ensure timely and effective care delivery.
- Prioritise urgent service and supply requests based on risk assessments and client needs.
- Seek management guidance on potential risks or issues identified when engaging with clients or staff within the Healthy Ageing at Home team.
- Ensure all interactions and record-keeping comply with national privacy and confidentiality requirements.
- Liaise with clients and families to explain HealthAbility services and support access to care.
- Work within defined role boundaries and escalate matters when outside scope of responsibility.
- Participate in task rotations alongside colleagues to ensure consistent service coverage.
- Ensure all service delivery is within program guidelines and aligned with healthAbility values.

Skills & Experience

- Experience working with the community aged care, community/public health or community-based setting.
- A good understanding of health conditions and health system including My Aged Care.
