About the Role The Intake Coordinator supports the General Manager in promoting the mlh Support at Home and Restorative Care programs through community engagement, marketing initiatives, and relationship development. The position plays a key role in converting enquiries and referrals into active participants through timely assessment, screening, funding activation and seamless onboarding.

Working closely with prospective participants, families, health professionals, community organisations and internal teams, you will ensure every consumer experiences a welcoming and professional transition into our services.

Position Objective The Intake Coordinator is responsible for supporting the growth and sustainability of Martin Luther Homes' (mlh) Support at Home and Restorative Care programs through effective community engagement, lead generation, participant intake and onboarding activities. Working closely with the General Manager, the role promotes mlh services within the community, develops and maintains referral relationships, and ensures prospective participants receive a positive, responsive and professional introduction to the organisation.

The Intake Coordinator is accountable for managing enquiries and referrals, conducting participant screening and assessment processes, coordinating My Aged Care and funding administration requirements, and facilitating seamless transitions into service. Through strong relationship management, customer service and administrative expertise, the role contributes to increased program participation,



organisational growth and positive participant outcomes while ensuring compliance with relevant aged care funding and regulatory requirements.

Key Selection Criteria Essential / Desirable Skills and Experience

- Demonstrated experience within Support at Home, Home Care Packages or Commonwealth-funded aged care is desirable.
- Understanding of My Aged Care and Australian aged care programs; experience in using the My Aged Care and PRODA portals is desirable
- Demonstrated success in converting enquiries or leads into admission or service uptake
- Experience managing inquiries, onboarding or intake functions
- Strong community engagement and relationship-building skills
- Excellent communication and interpersonal skills, with demonstrated cultural sensitivity and respect for diversity
- Ability to work both independently and collaboratively within a multidisciplinary team
- Proficiency in IT systems and digital tools used for care planning and documentation
- Current driver’s licence and willingness to travel within the service area




- Qualifications in Community Services, Aged Care, Business Administration or related discipline is desirable
- Clinical background highly regarded
