About the role

This is a part time, permanent position, 4 days a week, 9am to 5pm, based at the Concord office, with travel to participants across Sydney. You'll hold a caseload of participants, helping them understand their plan, choose their providers and get their supports up and running.

Key responsibilities

- Meet participants and families, work through their plan and set up the supports it funds

- Source and connect providers, chase waitlists and follow up until services are in place

- Track plan budgets so funding lasts the length of the plan

- Be the point of contact when something falls over — a provider stops turning up, a support isn't working, circumstances change

- Support participants through plan reassessments, including gathering evidence and reports

- Keep accurate case notes and billing, and work within NDIS Practice Standards

- Support participants to make their own choices about providers, including providers outside Aus Care Community Services

About you

- Experience in NDIS Support Coordination, or strong case management experience in disability, aged care, community services or a related sector

- Working knowledge of the NDIS — plan structure,



budgets, and what the NDIA does and doesn't fund
- A qualification in community services, social work, allied health, nursing or similar (Certificate IV or above), or equivalent experience

- Clear written communication and good record keeping

- Current NDIS Worker Screening Check, Working with Children Check, driver's licence and comprehensively insured vehicle

- NDIS Worker Orientation Module and First Aid, or willingness to obtain
