As a Residential Care Officer, you will deliver person-centered support, helping clients achieve their goals while ensuring their safety, wellbeing, and social connection. nKey Responsibility and Duties n
nClient Assessment & Program Development n
n• Conduct thorough in-home assessments to identify clients’ physical, emotional, and social needs, then design individualized support, training, and educational programs. n
n• Interview clients and their families to evaluate the nature and extent of difficulties and collaboratively develop goal-oriented care plans. n
n
nMonitoring, Referral & Reporting n
n• Regularly monitor client progress against care plans, documenting outcomes, and adjusting interventions as required to promote independence. n
n• Refer clients to appropriate health, welfare, housing,



or employment agencies to access additional services and resources. n
n
nCommunity Liaison & Support n
n• Liaise with community groups, government bodies, and private providers to raise awareness of available resources and address local service gaps. n
n• Support families by providing education, respite, and guidance for caring for children or adults with disabilities in home and group settings. n
n
nSupervision & Crisis Assistance n
n• Assist young people and vulnerable adults to resolve social, emotional, or financial challenges through practical advice and advocacy. n
n• Prepare funding submissions and detailed reports for government and funding bodies to secure resources for client programs. n
n
nRequirements n
n• 1 year of full-time work experience within community services, aged care, or disability. n
n• Relevant qualifications are required. n
n• Robust communication, documentation, and interpersonal skills. n
n• Valid driver’s license and willingness to travel to client homes. n
n
