Hawkesbury District Health Service is a teaching hospital in Windsor NSW, providing acute, community and allied health services for the local community and surrounding districts. The service provides 24-hour emergency, medical, surgical, diagnostic, maternity, neonatal, paediatric, palliative, intensive and coronary care services.
Join a dedicated healthcare team where your expertise can make a real difference to patient care. As an integral member of the multidisciplinary patient care team, you will provide a clinical pharmacy service that promotes medication safety and supports best-practice medicines management across all transitions of care, from hospital admission through to discharge.
Working closely with clinicians, community pharmacists and general practitioners, you will help ensure continuity of care and optimal outcomes for patients. In this role, you will provide expert therapeutic advice, undertake clinical medication reviews, and support patients and their families to use medicines safely and effectively. You will also contribute to medication safety and quality improvement initiatives, helping to enhance the delivery of high-quality, patient-centred care across the service.

What you will bring to the role
1. Tertiary qualifications in Pharmacy and current registration with the Australian Health Practitioner Regulation Authority (AHPRA).
2. Post graduate clinical experience in a healthcare setting
3. Excellent interpersonal, written, and oral communication skills.
4. Ability to prioritise workloads, work independently and within an inter-professional team environment.
5. Demonstrated high level of organisational and time management skills.
6. Record of ongoing professional development.
7. Computer literacy with experience in Microsoft Office (Microsoft Word, PowerPoint, Excel, and/or Access) and Pharmacy Inventory and Dispensing programs.
