About the role

You will work as part of an integrated multidisciplinary Allied Health team delivering person-centred services across aged care, restorative and community environments.

Key responsibilities

- Mobility & transfer assessment
- Balance & falls risk assessments
- Multiple outcome measure administration i.e DEMMI, SPPB for tracking functional progress
- Individualised rehabilitation with graded loading, falls-prevention strategies and group exercise class facilitation
- Post falls review
- Working collaboratively in a multidisciplinary team, including attending multidisciplinary case conferences alongside doctors, pharmacists and allied health staff, contributing to discharge planning, care pathways and clinical decision-making
- Manual handling assessments and care staff education
- Reablement and restorative interventions

About you

- Work experience in community aged care
- Experience working with TCP clients is desirable but not essential
- Passionate about older people and quality care

- Registered with AHPRA
