About the Role

The overall purpose of the Practitioner - Out of Home Care is to provide case management to children or young people within residential care services or programs. As a Practitioner – Out of Home Care, you will provide case management and holistic support to children and young people in residential care, helping them achieve positive outcomes through education, life skills development, emotional wellbeing, family connections and community participation. Working collaboratively with families, carers and key stakeholders, you will play a vital role in creating safe, stable and nurturing environments where young people can thrive.

This is a full-time opportunity based in Hervey Bay working 38 hours per week.

Pay rate of $53.42 per hour plus additional employee benefits.

Responsibilities:

- Provide comprehensive case management and ongoing support to children and young people in Out of Home Care.
- Develop, implement and regularly review individual case plans to achieve positive outcomes.
- Promote the physical, emotional,



social and developmental wellbeing of children and young people.
- Support children and young people to participate in education, employment, training and age-appropriate recreational activities.
- Build and maintain positive relationships with families, carers, schools and other key stakeholders.
- Advocate for the needs, rights and best interests of children and young people.
- Conduct risk assessments and develop strategies to ensure the safety and wellbeing of children and young people.
- Maintain accurate, timely and confidential case notes, reports and client records in accordance with legislative and organisational requirements.
- Collaborate with multidisciplinary teams and external service providers to coordinate effective support and achieve case plan goals.
- Work in accordance with Anglicare Southern Queensland's values, policies, child protection legislation and best practice standards to deliver high-quality services.

Qualifications, Credentials and Other Requirements

- Degree in Social Work, Psychology, Human Services, Behavioural Science or similar field
- Current QLD Drivers’ Licence & own reliable vehicle
- Current Paid Blue Card
- Current Licensed Care Service (LCS) or willing to apply
- Willingness to travel within service region
