The Program Lead, Social Support Group is accountable for Jewish Care's social support group program — what it offers, how well it runs, how many people it reaches, and what it costs. The program is delivered from the Active Living Centres at Caulfield and Windsor.
The role designs the program and leads the team that delivers it. It works alongside the Allied Health Practice Lead to build combined social and clinical offerings, so that participants can access group connection and clinical support through the same door.
The Active Living Centres are shared venues. The social support group program and the Allied Health team both work from them, and neither owns them. The Program Lead and the Allied Health Practice Lead are jointly responsible for making the Centres welcoming, safe and vibrant community hubs, and for bringing the social and clinical needs of participants and clients together.
The Program Lead carries the growth agenda for the program. That means building referral pathways with Care Partners and the community, leading marketing and communications, meeting agreed participation targets, and reporting on performance to Community Aged Care leadership.
This is the senior role in the social support group program.
