This role provides the opportunity to deliver high-quality social work services within a large tertiary hospital, supporting individuals and families through some of the most challenging periods of their healthcare journey.Working across inpatient and outpatient settings, you will provide psychosocial assessment, risk management and evidence-based interventions that support positive patient outcomes and continuity of care. The role offers exposure to a diverse and complex caseload, including patients affected by life-threatening illness, grief and loss, while working within a highly skilled multidisciplinary team committed to delivering compassionate, patient-centred care.**About you**Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:* Deliver high-quality social work services through comprehensive psychosocial assessment,
