About the role

Weipa Community Care Incorporated is seeking a Radio Case Manager (Youth Alcohol & Other Drugs) to join our team on a contract basis. Based in Weipa, QLD 4874, this role offers a unique opportunity to make a meaningful difference in the lives of young people struggling with substance abuse issues.

As Radio Case

Manager, you will provide essential support, advocacy and case management services to youth accessing our alcohol and other drugs programmes. This position is integral to our mission of delivering compassionate, evidence-based care to vulnerable young people in our community.

Key responsibilities

- Conduct comprehensive assessments of young people's alcohol and other drugs usage, identifying immediate and long-term support needs
- Develop individualised case management plans in collaboration with clients, their families and multidisciplinary teams
- Provide crisis intervention and support services to young people experiencing acute substance-related difficulties
- Facilitate access to detoxification, rehabilitation and other treatment services, including specialist referrals
- Establish and maintain therapeutic relationships with clients, offering motivational interviewing and evidence-based counselling techniques
- Coordinate care across external agencies and service providers to ensure seamless support pathways
- Monitor client progress against agreed goals and adjust support plans as required
- Maintain accurate, confidential and contemporaneous case notes and documentation in accordance with regulatory requirements
- Participate in team meetings, clinical supervision and professional development activities
- Advocate on behalf of young people with schools, families, legal representatives and other stakeholders
- Contribute to programme evaluation and quality improvement initiatives

What we're looking for

- Recognised qualification in social work, counselling,



psychology, nursing or a related discipline, with tertiary-level training in mental health, substance abuse or youth services
- Minimum 2-3 years' experience working with young people, preferably in youth mental health, alcohol and other drugs services, or community health settings
- Demonstrated knowledge of adolescent development, risk factors associated with substance abuse, and evidence-based treatment approaches
- Robust communication and interpersonal skills, with the ability to build trust and rapport with vulnerable young people and their families
- Proficiency in case management practices, including assessment, planning, coordination and evaluation
- Sound understanding of confidentiality, duty of care and mandatory reporting obligations within Australian healthcare contexts
- Experience with crisis intervention and de-escalation techniques
- Proficiency in electronic record-keeping systems and Microsoft Office applications
- Current First Aid and CPR certification
- Valid Australian driving licence with access to reliable transport
- Willingness to undertake relevant professional development, training and clinical supervision
- Ability to work flexibly and respond to the changing needs of young people in crisis
- Preferred experience with Aboriginal and Torres Strait Islander young people and cultural competency training
