What you'll do:

- Conduct assessments, reviews, coordinate care plans for nursing interventions tailored to individual customer needs.
- Provide nursing interventions such as wound care, medication management, continence care, and management of chronic conditions
- Observe and evaluate customer wellbeing, making timely and appropriate actions in response to clinical changes.
- Support the wellbeing of customers living in their own home

- Document nursing care accurately, thoroughly, and in a timely manner
- Participate in on-call after hours service
- Liaise with internal and external stakeholders including family members, Support Workers and Allied Health Professionals to support customers

About You!

You are a talented and passionate Registered Nurse who enjoys variety, values connection and wants to use your clinical skills in a meaningful way. You may already have community experience, or you may be ready to bring your nursing expertise into a new environment where every day looks a little different.

- Demonstrate significant experience as an Registered Nurse nb,



preferably in community setting
- Excellent attention to detail
- Excellent interpersonal, communication and problem-solving skills
- Ability to work autonomously
- Intermediate computer skills
- Availability to work on call
- Available to work at least 3 days per week

You will need:

- A nursing qualification and current Registered Nurse registration with AHPRA
- A current driver's license and reliable vehicle
- A satisfactory police clearance for working with vulnerable people that is less than 12 months old before commencement
