About Us We are a growing community care provider dedicated to delivering high-quality, person-centred care to older people living in their own homes.
We work closely with participants, families, Case Managers, and allied health professionals to provide coordinated, holistic care that supports independence, dignity, and quality of life.
Due to increasing demand from Mandarin-speaking participants, we are seeking a Registered Nurse who can provide culturally appropriate care and effective communication.
The Role
We are seeking a Registered Nurse (RN) with strong experience in clinical assessment and care planning, who enjoys working both independently and collaboratively in a community setting.
Key responsibilities include
- Conduct comprehensive health assessments
- Develop, implement, and review individualised care plans
- Provide ongoing clinical monitoring and support
- Deliver in-home nursing care as required
- Work closely with Case Managers to coordinate participant care
- Liaise with families, support workers, and allied health professionals
- Maintain accurate clinical documentation and ensure compliance
This role plays a key part in coordinating care alongside Case Managers to achieve holistic,
participant-centred outcomes.
About You
- Current AHPRA registration as a Division 1 Registered Nurse
- Experience in aged care or community care
- Demonstrated experience in:
- Clinical assessment
- Care planning and review
- Ability to work collaboratively with Case Managers and multidisciplinary teams
- Strong communication and organisational skills
- Ability to work independently in a community setting
- Mandarin speaking (preferred / highly desirable)
Requirements
- CPR Certificate
- Current Driver’s Licence and reliable vehicle
- Willingness to obtain:
- NDIS Worker Screening Clearance
