Join our Hospital in the Home team based at Newcastle Community Health Centre,

where you'll provide hospital-level care to patients in their own homes.

This isn't a role where every hour is scheduled for you. You'll be allocated a daily patient workload, then have the flexibility and professional autonomy to plan the most effective route, prioritise your visits, and structure your day.

A Day In The Life

- Deliver high-acuity hospital care in patients' homes
- Complete comprehensive nursing assessments and clinical observations
- Administer medications, provide wound care and collect pathology
- Support chronic disease management and patient education
- Liaise closely with HITH medical officers, GPs and the multidisciplinary team

You'll Be Someone Who

- Thinks critically and assesses situations with confidence
- Is comfortable as the sole clinician in a patient's home
- Communicates effectively and advocates for high-quality patient care
- Manages competing priorities with strong organisational skills
- A current Australian driver's licence
- A background in Emergency, Critical Care or Hospital in the Home is highly regarded.
