Job Description

Communify Queensland Ltd is recruiting for a role within its Medicare Mental Health Centre in Redcliffe, Queensland. You will join a multi-disciplinary team and provide psychosocial supports to assist participants in recovery and building independence through SMART goal setting.

The role involves outreach to participants in their homes and community, supporting them to attend appointments with services such as Centrelink and Hospital/Health.
