As a deeply grounded NDIS and Aged Care provider, we require Support Workers on a casual premise with a view toward permanency. One should be adaptable and flexible to work with individuals with diverse special needs and to help frail, older people, including the prematurely aged, living in the community to maximize their independence.

About the Role
Deivian Care is looking for a Service Coordinator in Clayton (VIC). The primary purpose of this position is to assist the Operations team in leading a team of Support Workers to deliver quality, person-centered and customer focussed outcomes for the people we support. Within the parameters of the role, the position is responsible for the quality of services, ensuring timeliness, and working within the agreed budget. The role is also responsible for implementing a planned approach to the leadership, supervision and professional development of the team members.
The primary function of this role is to support stakeholders (clients, client families, and employees) to achieve their individual goals and objectives in accordance with their participants Individual Care Plan.

Responsibilities:
- Carefully match and align support workers with clients to ensure compatibility, fostering long-term professional relationships that meet client needs and preferences.
- Coordinate day-to-day services, including scheduling, rescheduling, and communicating changes with both support workers and clients.
- Arranging meet & greet sessions between Clients and Support workers before commencing shifts. Any deviations require managerial approval.
- Participate in interviews for support workers based on staffing needs and coordinate any requirements with HR, including assigning selected candidates for staff induction. This includes meeting the prospective support worker face-to-face during the interview process.
- Communicate with HR regarding specific training requirements such as staff induction, BSP training, care plan-related training, or training for specific diagnoses.
- Manage weekly timesheets for support workers and client services, ensuring accuracy in hours, notes, kilometers, and other relevant details before submitting to payroll/accounts.
- Read, understand and communicate the clients profile such as their diagnosis, mobility requirements, specific needs to the Support Workers along with the task that needs to be completed. Eg, Domestic Cleaning, shopping. This also includes communicating information about date, time, location or other third party information.
- Liaise with the Accounts team to resolve discrepancies and address complex payment or invoicing matters related to time sheets.
- Ensure service agreements are signed for all current clients and that the complete onboarding process is compliant, with any deviations requiring managerial approval.
- Collaborate with support coordinators and care managers to ensure client budgets are adhered to, taking proactive steps as needed.
- Maintain positive relationships with third parties, including allied health professionals, GPs, Local Area Coordinators, external support workers, and other stakeholders, promoting the organization and maintaining a strong industry presence.
- Engage in relationship-building activities such as sending emails, greetings during special occasions, making calls, and meeting third parties to maintain professional connections.
- Attend industry-related events, establish new contacts, and follow up to maintain ongoing professional relationships.
- Document outcomes of connections, including emails sent and events attended,
in tracking systems or spreadsheets.
- Maintain accurate business metrics and prospective client reports as required by management.
- Ensure strict adherence to compliance in onboarding and offboarding processes, maintaining the recommended notice period for offboarding, and obtaining approvals for any deviations.
- Conduct quarterly client visits and support worker supervision checks to ensure satisfaction and maintain strong relationships. Ensuring both these activities are documented to ensure full compliance for client satisfaction and support workers performance management.
- Inform HR, Accounts, and Payroll of support worker resignations, provide resignation documentation, and ensure timely removal from the scheduling systems (QuickBooks).
- Ensuring Support Worker notes are reviewed daily to ensure all workers have written their notes, any concerns are picked up, and incidents are recognized. This is a mandatory process and ensures a well-being check of both support workers and clients on a daily basis.
- Regularly review client notes, including those from the last three months, to identify important information, including in the event of client deaths, while performing daily note-checking activities.
- Inform management, support coordinators, case managers, compliance officer, HR, administration, accounts and other relevant stakeholders about the death of clients within 24 hours.
- Follow established processes for incident reporting, complaints management, risk management, and other operational issues.
- Maintain confidentiality, emotional intelligence, professionalism, and high ethical standards in all interactions and activities.
- Participating in online and face-to-face trainings internally & externally as communicated by HR. Taking self-initiatives to stay updated about the industry standards and changes.
Experience/Certification Required
- Bachelor’s degree/Diploma in a relevant field
- Cert III or IV in Individual Support/ Ageing Support/Disability
- Experience in Aged Care | NDIS
- Full Victorian Driver's License and your own vehicle (registered in your name)
- National Criminal Check
- Working with Children Check
- Covid-19 Vaccination Certificate
