HiTalent Recruitment is seeking an experienced Mental Health Case Manager to support individuals and families impacted by mental health challenges. This role will work directly with people experiencing acute distress, trauma responses and mental health crisis to provide compassionate, structured support and ensure access to the right services at the right time. This is a 3 month full time temporary position starting immediately.
This is a highly sensitive and meaningful role suited to Case Managers who are calm, capable and experienced in working with trauma, crisis and complex presentations.
About the Role
As a Mental Health Case Manager, you will provide trauma-informed case management and psychosocial support, including:
- Supporting individuals directly affected by mental health and suicidality
- Conducting psychosocial assessments and risk screening
- Providing service navigation and referrals across mental health, trauma and practical supports
- Coordinating care with multidisciplinary stakeholders and external agencies
- Supporting people through stabilisation and recovery planning
- Maintaining clear documentation and care plans in line with best practice
Key Responsibilities
- Provide trauma-informed, client-centred case management
- Complete care coordination and psychosocial assessments and ongoing review
- Undertake risk screening and safety planning, escalating where required
- Coordinate referrals to specialist trauma counselling, mental health services and bereavement supports
- Support clients to access practical supports including legal, financial, housing and community services
- Advocate for clients to reduce barriers and improve service access
- Liaise with families, guardians, health services and other partners
- Maintain accurate documentation and contribute to reporting requirements
- Participate in reflective practice, case reviews and clinical supervision
What We’re Looking For To be successful in this role, you will have:
- A tertiary qualification in Social Work, Psychology, Counselling, Mental Health Nursing or similar
- Demonstrated experience supporting clients impacted by trauma, crisis and complex mental health
- Strong skills in psychosocial assessment, risk screening and safety planning
- Experience providing service navigation and coordinating across multiple systems
- High emotional intelligence and solid professional boundaries
- Excellent communication and documentation skills
- Ability to work independently and within a multidisciplinary response team
- A trauma-informed, culturally safe and non-judgmental approach
Requirements
- NSW Driver Licence (essential)
- Working With Children Check + National Police Check (or willingness to obtain)
- Availability to travel across Sydney as required
- Flexibility may be required during peak response periods
