Position: Community Support Worker

Salary: $82,320.16 per annum ($41.66 per hour, 38 hours per week), plus superannuation, allowances and bonus

Home Caring (Bankstown) Pty Ltd is a home care and disability support provider based at Chipping Norton, delivering in-home and community-based services to older Australians and NDIS participants across Bankstown, Liverpool and the wider South Western Sydney region. The business supports clients through Home Care Packages, Support at Home, NDIS in-home support, community participation, supported independent living and short-term accommodation, with care coordinated by an experienced team of case managers, registered nurses and team leaders and backed by a dedicated rostering and administration function. Home Caring is locally owned and operated with the support of a national network, and serves a strongly multicultural client base whose language, cultural and religious preferences shape the way every service is delivered. The organisation places its emphasis on person-centred practice, dignity, cultural safety and consistency of care, and supports its workers with structured supervision, clear escalation pathways and ongoing training in compliance and quality.

About the Position

The Community Support Worker delivers direct, person-centred support to clients in their own homes and in the community, working to individual care plans and under the direction of the relevant team leader, case manager or registered nurse. The role combines hands-on personal care and daily living support with practical assistance that helps clients maintain independence, stay connected to their community and work towards their own health, social and recovery goals. Because services are delivered one-on-one and often away from the office, the position calls for sound independent judgement, accurate documentation in the organisation's rostering and service-record systems, and the confidence to identify risk and escalate it promptly. It is a central role in a growing service that is accountable to the NDIS Practice Standards, the NDIS Code of Conduct and the strengthened Aged Care Quality Standards.

Key Responsibilities

- Provide high-quality, timely and person-centred support to clients in accordance with their individual care plans,



assessed needs and the directions of Home Caring's supervisors and clinical staff.
- Support clients with personal care and daily living activities, including hygiene, dressing, mobility, meal preparation, household tasks and other self-care activities.
- Provide practical and emotional support to clients and their families, promoting each client's independence, wellbeing, dignity, privacy and informed choice.
- Assist clients to identify and work towards their personal, social, health and recovery goals, and support them to build independent living and life skills.
- Provide clients with information about community services and resources available to them, and assist them to participate in community, social, recreational, educational and employment-related activities.
- Provide or arrange transport to medical appointments, community programs, shopping and social or recreational activities where authorised.
- Identify clients' needs and assist them to access appropriate health, welfare, housing, employment, training, mental health and community services.
- Identify and promptly report risks and urgent concerns, including suspected abuse, neglect, homelessness, mental health crises and medical emergencies, and respond in accordance with Home Caring's procedures.
- Deliver all services in a culturally safe, respectful and appropriate manner, having regard to each client's background, language, beliefs and preferences.
- Provide medication assistance only where authorised, and strictly in accordance with the client's care plan, relevant legislation, training requirements and Home Caring's policies and procedures.
- Comply with applicable food safety and handling requirements when purchasing, preparing, storing or serving food.
- Comply with work health and safety requirements, infection-control procedures, manual-handling practices and all other relevant safety standards.
- Maintain accurate, complete and timely records of services provided, client progress, observations, incidents and any matters affecting service outcomes.




- Promptly report changes in a client's health, behaviour, circumstances or support needs to the appropriate supervisor or health professional.
- Maintain professional and respectful boundaries with clients and their families while supporting dignity of risk, informed decision-making and client choice.
- Communicate and cooperate with clients, families, supervisors, health practitioners and other service providers to support continuity of care.

Requirements

- Australian citizens and permanent residents are encouraged to apply.
- Diploma of Community Services.
- A minimum of two years' relevant experience in community, disability or aged care support work.
- Current First Aid and CPR certification.
- NDIS Worker Screening Clearance, a National Police Check and a Working With Children Check, or the ability to obtain them prior to commencement.
- A current Australian driver's licence and access to a registered, insured and roadworthy vehicle, as services are delivered across multiple client homes and community locations.
- Sound working knowledge of the NDIS Practice Standards, the NDIS Code of Conduct and the Aged Care Quality Standards, and of infection control, manual handling, food safety and work health and safety requirements.
- Practical competence in personal care, domestic assistance, meal preparation and daily living support, including the safe use of mobility and transfer equipment.
- Confident use of digital rostering and client record systems for clock in and out, progress notes, shift reports and incident reporting.
- Understanding of privacy, confidentiality and safeguarding obligations, and of mandatory and internal reporting requirements.
- Robust communication and active listening skills, with the ability to work effectively with clients from culturally and linguistically diverse backgrounds; a second community language is an advantage.
- Empathy, patience and respect, together with sound judgement, reliability, flexibility and the ability to work independently in clients' homes as well as collaboratively with families, colleagues and health professionals.
- Availability to work a rotating roster across weekdays, evenings and weekends in line with client needs.
