Are you an experienced Care Partner with a background in community aged care?

Workforce Extensions Sunshine Health is seeking a compassionate and reliable Care Partner for our client, Solara Health. If you have experience supporting older people in their homes and are passionate about providing high-quality, person-centered support, we’d love to hear from you.

What do you need?

- Qualifications or experience in Nursing is preferred

- Experience in Case Management is preferred

- Robust time management and organisational skills

- Excellent verbal and written communication

- Ability to build positive relationships with clients, families, and advocates

- Ability to work autonomously and in a team environment

- Current Police Check, driver’s licence

Duties & Resposibilities

- Provide end-to-end support for clients receiving Support at Home Packages, including assessment and commencement

- Work with clients to identify strengths and abilities and deliver appropriate care and services

- Develop, review,



and update care plans to support client goals

- Liaise with medical, allied health, and community stakeholders

- Maintain regular client contact, including monthly monitoring calls

- Implement and manage client budgets

- Complete administrative tasks, including case notes

- Ensure compliance with Aged Care Quality Standards

- Speaking a second language is an advantage

Extra Information

- Vehicle provided/own: company vehicle provided

- Pay rates: $80K + super

- Reporting to: Community Care Manager
