About the role
Provide practical, person-centred assistance to people with disability in their homes and community. Support personal care, mobility, meals, household activities, social participation and daily routines while respecting each participant's choices, culture, rights and independence. Implement established support plans and report changes; do not assume clinical, case-management or funding authority.
Key responsibilities
- Assist with showering, bathing, grooming, dressing, toileting and continence support as allocated. Explain the support, seek consent, protect privacy and encourage the person to do what they can safely do independently.
- Support walking, positioning and transfers using assessed techniques and equipment. Use hoists, slings and wheelchair equipment only after competency and participant-specific instruction are verified; follow required staffing arrangements.
- Help with grocery shopping, meal planning, preparation, eating and drinking in accordance with the current mealtime plan. Complete participant-related cleaning, laundry and household tasks. Follow hygiene, allergy, texture and swallowing instructions; report uncertainty or changed needs.
- Accompany participants to appointments, shopping, social and recreational activities. Support communication, relationships and daily living skills. Drive authorised vehicles safely, complete pre-start checks and follow passenger and wheelchair-transport procedures.
- Provide respectful companionship and emotional support without creating dependency or crossing professional boundaries. Use accessible communication and support informed choices. Deliver culturally responsive support, including for Aboriginal and Torres Strait Islander participants.
- Observe wellbeing and follow current health and medication instructions.
Assist or administer medication only within verified training, competency and authorisation. Complete medication records and immediately escalate errors, refusals, missed doses, adverse reactions and health deterioration.
- Implement current positive behaviour support strategies and recognise known triggers. Use proactive, least-restrictive support; do not invent restrictions or change a behaviour support plan. Report actual or suspected abuse, neglect, exploitation, discrimination, violence and unauthorised restrictive practices immediately.
- Read current support, risk, health, communication and behaviour plans before the relevant work. Complete factual ShiftCare notes, medication records, incident reports and other required documents promptly. Give transparent handover and escalate missing information, changed needs and unresolved safety issues.
- Follow infection control, manual handling, mealtime, transport, lone-work, heat and fatigue controls. Report hazards, near misses, aggression and unsafe staffing or tasks. Safeguard confidential information and participant money/property; do not borrow, lend or conduct personal financial transactions with participants.
- Attend paid induction, supervision, team meetings, toolbox talks and required training. Maintain screening, licences and competencies. Notify management promptly about absence or inability to work safely, and contribute to continuous improvement.
