Disability Support Worker – SIL, Home Care & Community Participation

Nurture N Revive is looking for caring, reliable and motivated Disability Support Workers to join our growing team.

We provide a mix of Supported Independent Living (SIL), Home Care and Community Participation supports, so no two days are necessarily the same.

Location: Melbourne South-East

The role

You may be supporting participants with:
Daily living and household tasks in SIL
Personal and social support
Community access and participation
Shopping, appointments and outings
Recreation, hobbies and social activities
Meal preparation and everyday routines
Supporting independence, confidence and community connections
Daily support documentation and routines

What we're looking for
Previous disability support experience preferred
Current driver's licence and your own reliable car – essential
Comfortable and confident driving participants
Caring, patient, respectful and reliable
Good communication skills
Versatile and willing to work across SIL,



Home Care and Community Participation
Ability to work independently and as part of a team
Relevant NDIS screening, First Aid/CPR and other required checks
