Independent Support Services are currently looking for a Team Facilitator to support our Northern Suburbs team. This full time permanent role blends direct customer support with team leadership and coordination to work within a small family owned business supporting people living with disabilities in the community.

You’ll work closely with Support Workers and the Director to ensure participants receive protected, consistent, and high-quality services.

This role is a hybrid role that includes:
Working directly with participants in the community and their homes
Providing coaching, mentoring, and supervision to Support Workers
Handling record keeping for NDIS and stakeholders.
Part of the role is support work and part is administration.

What you’ll do

You’ll be supporting participants with a range of needs, Mental Health, ASD MS and Parkinson’s disease. Duties may include:
Personal care, domestic assistance, mental health support, and community access.
Working alongside Participants to build their capacity.
Building strong relationships with customers, their families, and the support team
Provide practical, social,



and community-based support
Ensure support plans and documentation are accurate and up to date.

· What you will need for the role:
A valid driver’s licence and reliable vehicle
NDIS Worker Screening Check (or willingness to obtain)
Working with Children Check (or willingness to obtain)
Current First Aid & CPR
Certificate IV in Disability (or working towards)
Minimum 2 years’ experience as a Support Worker.

You will need to have the skills to:
Ability to lead, motivate, and support a team
Have a clear understanding of the English language both written and verbal.
Ability to set boundaries while providing compassionate care.
Ability to work part of a team and autonomously
Ability to complete admin tasks from home or at the office.

Job Types: Full time, Part-time

Pay: $38.00 – $41.00 per hour

Expected hours: 25.0 – 38.0 per week
