SNG Healthcare Solutions is seeking an experienced Disability Support Worker to provide individualised support to a 17-year-old participant living in Warnbro, Perth WA.

The role includes:
Providing individualised support at home and in the community
Supporting meaningful activities, outings and daily routines
Encouraging independence, choice and confidence
Building a trusting and respectful support relationship




Working positively with family and the broader support team
Following individualised support strategies and organisational procedures
Maintaining skilled boundaries, confidentiality and accurate documentation

Hours & Work Monday to Thursday, 11:00am–2:00pm

Ongoing, consistent shifts with the opportunity for additional flexible hours and a focus on building a secure and positive support relationship.

Requirements (or Willingness to Obtain):
NDIS Worker Screening Check
First Aid & CPR
Current Australian Driver’s Licence and reliable vehicle
Good English communication skills
