The Royal Melbourne Hospital is seeking a Diabetes Educator to join the HARP Diabetes Co-Management Service.
This part time clinical role ****** hours per fortnight) will deliver care across clinic, home-based and telehealth settings for people with unstable diabetes who are at risk of hospital admission.You will work with consumers, families, GPs and RMH teams, providing diabetes education and self-management support, underpinned by advanced nursing practice and specialised education.
