HammondCare is recruiting 3 x 0.6FTE Registered Nurses to support clients across the Central West, Nepean and Blue Mountains region. This permanent part-time role involves home visits, group activities and delivering patient-centred care in clients' homes.

You will work within the Home Care Clinical Care & Practice Team, collaborating with a multidisciplinary team to promote independence and quality of life for older adults receiving Support at Home or CHSP services, with adaptable hybrid working
