What you will do
Deliver high-quality clinical pharmacy services across hospital wards.
Conduct comprehensive medicines reconciliation and medication history assessments.




Perform clinical screening of prescriptions and drug charts to ensure patient safety.
Complete TTA screening and detailed discharge medication reviews.
Provide specialist pharmaceutical advice to multidisciplinary healthcare teams.
Support safe and effective medicines optimisation across the service.
Review and develop medication therapy plans to improve patient outcomes.
Participate in ward rounds and contribute to clinical decision‐making processes.
Ensure adherence to medicines management policies, procedures, and best practice guidance.
Maintain accurate clinical documentation and pharmacy records.
Requirements
GPhC Registration:
Qualified Pharmacist with current General Pharmaceutical Council registration.
NHS Experience:
Proven NHS experience, particularly within hospital ward-based pharmacy services.
Medicines Reconciliation:
Experience completing drug histories and medicines reconciliation.
Clinical Screening:
Strong experience with clinical screening and TTA reviews.
EPMA Knowledge:
Familiarity with EPMA systems, preferably Cerner EPMA.
Clinical Expertise:
Excellent clinical pharmacy knowledge and decision-making skills.
Communication Skills:
Strong interpersonal, organisational, and multidisciplinary working skills.
Band 7 or Band 8a Experience:
Previous experience working at Band 7 or Band 8a level.
Acute Hospital Experience:
Experience within an acute hospital pharmacy environment.
