Position located at Heidelberg Repatriation Hospital, working with our clients in the community.
Part time fixed term 12 month position
Exciting opportunity to trial a Nurse Practitioner led model for the Community Transition Care Program

About the Transition and Restorative Care Programs
The Transition and Restorative Care Programs sit within Aged Care Community Services as part of the Continuing Care Division at Austin Health.
The programs are committed to providing high quality safe care for patients while helping them achieve their goals.
The programs are provided in both residential and community settings.
About the Role:
Nurse Practitioners (NP) are supported by Austin Health to work in clinical specialties where there is evidence that the advanced clinical skills of a Nurse Practitioner would enhance patient outcomes within an efficient service model.
The Nurse Practitioner works with the broader Transition Care Program team to enhance the program's capacity to respond to increasing client complexity by providing timely,



in-house management of medical and complex nursing needs for clients in the community based program.
The Nurse Practitioner role will support rapid clinical assessment and intervention; improved oversight and governance of associated nursing providers and enhanced care coordination across the TCP multidisciplinary team.
By strengthening internal clinical capability, the Nurse Practitioner is expected to reduce preventable emergency department presentations, support client complexity, improve discharge planning processes, and support compliance with the Aged Care Quality Standards.

You will be someone who has excellent communication skills, with a strong commitment to excellence in patient care.
You'll bring energy and drive to your work, and have a keen interest in developing your skillset.
You'll further bring with you:
Current AHPRA registration and Nursing & Midwifery Board of Australia endorsement as a Nurse Practitioner
Extensive and recent clinical experience in Aged Care
Hold a valid Australian prescriber number
Demonstrated commitment to ongoing personal and professional development, as evidenced in a Continuing Professional Development (CPD) record/Professional Practice Portfolio (PPP)




Excellent interpersonal and communication skills that support patient care, team engagement and collaboration
